709.93(a)(11) LICENSURE Client records
709.93. Client records.
(a) There shall be a complete client record on an individual which includes information relative to the client's involvement with the project. This shall include, but not be limited to, the following:
(11) Follow-up information.
|
Observations Based on the review of client records, the facility failed to document that follow-up was provided to the client per agency policy in one of two client records. Follow-up documentation was missing from client record #6.
|
Plan of Correction This situation occurred as a result of an electrical fire in the office that damaged the main computer. Because of the damage, client data could not be be retreived until a new computer was obtained and the hard-drive with the data bank was transferred over to the new computer. This process took several weeks, but has been completed. An external back-up drive was also installed, so that if an event like this occurrs again (as long as that back-up drive is not damaged), it can be plugged into any computer to retreive this data. Follow-up is again being completed in a timely manner. |
