This report is a result of an on-site licensure renewal inspection conducted on April 14, 2025, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Oil Region Recovery, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Observations
Based on a review of client records, the facility failed to complete treatment plan updates at least every 60 days in four of six applicable outpatient records reviewed.
Client #15 was admitted November 27, 2024 and discharged February 24, 2025. A treatment plan was completed on December 10, 2024 and an update was due by February 8, 2025 but not completed prior to discharge.
Client #17 was admitted October 30, 2024 and discharged January 20, 2025. A treatment plan was completed on October 30, 2024 and an update was due by December 29, 2024 but not completed prior to discharge.
Client #18 was admitted August 20, 2024 and discharged February 20, 2025. A treatment plan was completed on September 3, 2024 and an update was due by November 2, 2024 but not completed until November 5, 2024. Another treatment plan was due by January 4, 2025 but not completed until January 9, 2025.
Client #21 was admitted November 7, 2024 and still a current client. A treatment plan was completed on November 25, 2024 and an update was due by January 24, 2025 but not completed until February 5, 2025.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
Treatment plans are to be updated every 60 days in OP level of care. If a client doesn't show for a appointment when treatment plan is due to be updated, the counselor will complete the update and sign and date the document. The next appointment the client attends they will sign the document and update any additional goals/objectives that they want added to the treatment plan. This will be followed by the Clinical Director and implemented on 4/30/25. |
