This report is a result of an inspection of the addition of partial hospitilization conducted on May 8, 2025, by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment, Program Licensure Division. Based on the findings of the on-site inspection, Synergy Treatment LLC was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
Since this was the initial inspection, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.
