Three personnel records were reviewed on November 16, 2015. Three personnel records were required to have documentation of HIV/AIDS and TB/STD training. The facility did not provide documentation of the required HIV/AIDS and TB/STD training in personnel records # 2.

Employee # 2 is a counselor and was hired April 7, 1997. HIV/AIDS and TB/STD training was due to be completed no later than April 7, 1998. The facility failed to provide documentation of HIV/AIDS and TB/STD training.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
This was not reviewed with the staff a the time of the visit. If it had I would have been able to produce these records. This was never a citation in the past as this employee had HIV training over 10 years ago. To prevent this confusion in the future the director will place mandatory trainings in a distinct section of the chart for easy accessibility and identification. this process has begun and will be the sole responsibility of the director and will be completed within the first year of a new hires employment.

Employee received trainings on 4/7/97 with Arelis Sable. 6 hours HIV/AIDS, and 4 hours TB/STD. Documented in all staffing requirement forms since 1997.
