Observations Based on a physical plant inspection, the facility failed to keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times. The ceiling in the living room had a large hole with exposed pipes and had been this way for several weeks.
Also, a bedroom door on the third floor did not have a doorknob or any way of keeping the door closed.
|
Plan of Correction Regular visits will be made during times of repair by the District Manager to ensure repairs are done in an expedited manner.
All repairs have been made. The ceiling is fixed and the door knob is replaced. |
