Based on a review of the Staffing Requirements Facility Summary Report form completed by the facility on July 23, 2013, the facility failed to ensure that staff caseloads remained at or under 35:1.

The findings include:

The Staffing Requirements Facility Summary Report form completed by the facility was reviewed on July 25, 2013. The form listed one facility/clinical supervisor and three counselors.

Employee #4 has documented 10 hours dedicated to drug and alcohol clients, based on a 35 hour work week. Employee #4 had a case load of 12 drug and alcohol clients at the time of inspection. Employee #4 exceeded the required 35:1 FTE and currently had a FTE of 43:1.

Plan of Correction
The Clinical Supervisor met with the staff on September 5, 2013. A review of the staffing requirements was done with staff at that time.

Employee #4 has been instructed to get her caseload into compliance by September 30. She is going to complete treatment with those that are ready for discharge so as not to disrupt the treatment process. She will not be assigned any further clients until this is accomplished and she is in compliance. Employee number 4 has also agreed to increase her work hours to 15 per week.

The Clinical Supervisor will review her progress weekly through September 30. Her active caseload will be reviewed on a by-weekly basis after September 30 to ensure that we are compliant with this standard.
