The cook will continue recording temperatures of all refrigerators and freezers twice daily on the facility temperature logs. The cook will immediately report any refrigerator temperature above 40 degrees, and any freezer recording above 0 degrees. 709.28 (c) LICENSURE Confidentiality
§ 709.28. Confidentiality.
(c) The project shall obtain an informed and voluntary consent from the client for the disclosure of information contained in the client record.

Additionally, 1 of 14 client records had consent to release forms that were missing the expiration date and/or witness signature.
Client #11 was admitted on 6/2/17 and was still an active client at the time of the inspection. There were consent to release forms signed and dated by the client on 6/1/17 to four different treatment providers and a government agency, but they did not include the witness signature and expiration date. Additionally, there was a consent to release form signed and dated 6/2/17 to a healthcare provider that was not specified, nor did it have a witness signature.
Client #16 was admitted on 4/21/17 and was discharged on 4/26/17. There was documentation of correspondence with a government agency on 5/25/17; however, there was no consent to release information form signed by the client prior to the disclosure of information.

These findings were reviewed with facility staff during the licensing process
