Observations
Based on review of the provided records the recovery house failed to provide a written notification to the resident.

Plan of Correction
The House Manager has developed a process with the House Lead for individuals asked to leave the recovery house. On October 29th, the House Manager led a training for the House Leads regarding proper protocol for asking for ending residency for residents.

The House Lead will call the House Manager to alert them of any need to ask a resident to leave the house. If the House Manager approves, then the House Lead will make the resident aware of why and when the resident needs to leave the recovery house. The House Lead will prepare the Notice to End Residency form. The House Lead will fill out the form with the resident's name and reason for termination. The House Lead will ask the resident to sign the agreement and note if they refuse. Upon filling out the form, the House Lead will contact the emergency contact and mark on the form that it was completed. If they accept, the form will be copied and provided to the resident. The form will be submitted to the House Manager to scan and put in the resident folder. If the resident refuses to accept the form, the House Manager will email the resident a copy of the email provided to the Recovery House.
