Based on client record review, the facility failed to document a treatment plan update in one of one client records.
The findings include:
Five client records were reviewed on March 9, 2009. A treatment plan update was required in one client record. The facility did not document a treatment plan update in client record # 3.

Plan of Correction
Pathway to Recovery, Clinical Supervisors will review outcome of site visit and documentation standards with regards to time frames for required treatment plan updates with the addiction counselors during a staff training/meeting on March 17, 2009. Treatment plan update for client record #3 is completed and filed in the chart. Implementation date of March 17, 2009, will reflect compliance with regulation. The Facility Director and Clinical Supervisors will follow up and monitor to ensure ongoing compliance with the regulations. |
