For the next 9 months, the counselor assistant may counsel clients only under the close supervision of a lead counselor or a clinical supervisor.
Observations
Based on a review of personnel records and an interview with the clinical supervisor the project failed to document close supervision requirements, to include documentation of one hour of direct observation per week, in one of one counselor assistants (trainees) supervisory documentation. One counselor assistant possesses a bachelor degree without a year of clinical experience.

Findings:

Record review # 2 - A counselor assistant was hired on January 11, 2010. Based on the qualifications documented in the personnel record on February 16, 2010, the counselor is to be supervised for at least 6 months under close supervision, which is to include at least one hour of direct observation per week. A review of the monthly evaluation form in the personnel record revealed that the facility failed to document the dates of the weekly hour of direct observation.

Plan of Correction
For the next six months the Outpatient Supervisor will utilize our established Probationary Evaluation Form which has been updated in the Feedback section to include weekly dates and signatures of times that direct supervision occurred. The Outpatient Supervisor will utilize the GroupWise Calendar System to schedule the weekly times for direct supervision with the two trainees. At the time of the direct supervision the Probationary Evaluation will be reviewed in addition to review of files checked for the week and any issues and questions the trainees have. The Outpatient Supervisor will fill in the dates and sign the supervision section and forward to the Administrative Assistant who will file in the Personnel Binders of each trainee. The first weekly supervision will occur during the week of March 1, 2010. This will occur for the next six months until each trainee reaches one year of direct experience
