INITIAL COMMENTS
 
This report is a result of on-site unusual incident investigations conducted on September 16, 2024 by staff from the Bureau of Program Operations. Based on the findings of the on-site investigation, New Castle Comprehensive Treatment Center was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
