This report is a result of an off-site complaint investigation conducted on March 11, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, 25VZ6701 Safe Haven in York PA Inc was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the recovery house. Therefore, no plan of correction is required.
