Findings:

The White Deer Run WEB site advertises educational services for their adolescent clients.

Client # 1 was admitted on March 1, 2011 and discharged on March 11, 2011. Upon his arrival to the facility, no arrangements had been made to involve the client in an educational program.

An interview was held with employee #1. Employee # 1 stated that no one knew anything about him, (referring to the adolescent program staff). The client was 18 years old and he was sent to the adult unit.

Employee # 1 acknowledged that the school called the facility on March 11, 2011, but the client was already transferring out of the facility. Employee # 1 also believes that the school called the facility prior to that on March 4, 2011, however staff were unable to acknowledge the client's presence to the school since consent to release information forms had not been executed by the client permitting the facility to release information to the school. As a result, services could not be coordinated between the client's school and the facility.

An interview was conducted with employee #2, who was the client's primary counselor. Employee # 2 stated he/she was not advised that client #1 was still in school. Employee #2 states his/her first interaction with the client was on March 7, 2011 for an assessment. Employee # 2 stated that without consents to release information from client # 1, he/she could not have known that client # 1 was still enrolled. Employee # 2 advised that this was work that was usually completed during the intake process." Additionally, Employee # 2 described a lengthy conversation with the client and later with the client's mother on March 8, 2011 when the client attempted to leave the facility against program advice. Employee #2 said at no time did education or the client ' s schooling come up.

The client's record was reviewed on March 16, 2011. There was no documentation regarding educational services in the client ' s record.

Plan of Correction
In review of the patient's record and interviews with staff, there was clearly an oversight during the intake process in which an assessment of the patient's educational status and the necessary consents were not obtained. The Nursing Assessment clearly identifies that the patient is in 12th grade and a communication from the patient's counselor to the UR Manager for this patient indicated that a discussion about his education plan had occurred. It is the practice of White Deer Run Allenwood to provide one hour of instruction and tutoring with the contracted Intermediate Unit licensed teacher consistent with the Warrior Run School District calendar, for those patients who are in adult units and are still in high school.

The performance deficiency by the Intake Technician has been addressed through supervision and counseling by the Director of Intake and Medical Records on April 12, 2011. A policy and procedure review with the Intake Department by the Director of Intake and Medical Records is scheduled for the week of April 18th. The Clinical Supervisors of the Adult Units have received communication from the Regional Director April 12, 2011 regarding the need to assess educational status for those under the age of 21 and arrange for educational services with the Intermediate Unit teacher.

The intake supervisor will review all previous day intakes for those under the age of 21 years old to determine if educational status was defined. In cases where it was not, the intake supervisor will contact the appropriate home school district after having obtained the necessary patient consent and seek payment authorization from the school as well as planned educational activities and homework assignments. The intake supervisor will also contact the Unit Supervisor of the respective patient and notify him/her of the need for the patient to included in educational services either in the regular educational program for minors or the 1 hour tutorial educational service for adults.
