This report is a result of an on-site recovery house complaint investigation conducted on June 24, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, TNH D8YH6701 was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house. An appropriate plan of correction has already been implemented at the time of inspection and all repairs have been made. No additional POC will be required.
