Plan of Correction A death occurred at the licensed Lancaster House located at 131 East Orange Street. Although the county SCA was notified of the incident, it was later discovered that no report was sent to DDAP. The plan moving forward is to review all pertinent policies and procedures with the current Facility Director who will then do the same with the House Manager and other staff members in the Lancaster office. This will ensure that that all other individuals involved with running the RASE House recovery houses are aware of proper procedures in the event of a death. Additionally, the recovery house is currently under contract for sale no later than March 31, 2026. Once the property is sold, RASE Project will not be operating any recovery houses ever again. |
