This report is a result of an off-site complaint investigation conducted on 6/24/2024, by staff from the Bureau of Program Licensure. Based on the findings of the off-site complaint investigation, G0B96701 C And A Solutions was found not to be in compliance with the applicable chapters of 28 PA Code Chapter 717 which pertain to the recovery house, a Plan of Correction is required.

§ 717.20(b) LICENSURE Resident Records
§ 717.20. Resident records.
(b) The licensee shall keep hard copy resident records in a locked cabinet and secure digital resident records on a protected data system.

Observations
Following the request to review resident records on June 24, 2024, it was found that the recovery house failed to keep hard copy of resident records in a locked cabinet and secure digital resident records on a protected data system. A Plan of Correction is required.

Plan of Correction
On 7/4/2024 the owner /House Manager will keep files in a locked and secured area. Owner will go back to utilizing one step recovery program so that both digital and paper forms are always safe and secure. Owner will make sure every client is given all forms required to fill out upon arrival and transported back to the business office where it will be stored under surveillance by ADT starting 7/4/2024 |
§ 717.23 LICENSURE Notification of decision to end residency
§ 717.23. Notification of decision to end residency.
(a) The licensee shall notify the resident in writing of a decision to end residency. The notice must include the reason and a timeframe for ending residency.
(b) The resident shall have an opportunity to request the licensee reconsider a decision to end residency before the decision to end residency takes effect.
(c) The resident may decide to end residency without providing a reason to the licensee.

Plan of Correction
Plan of correction. All residence will be given the end ofresidency when being discharged. This will start immediately as of 7/42024. All paperwork will be filled out immediately by clients upon arrival and kept at the secured business address of 311 walker ave. Butler pa 16001. All files will be under video surveillance at all times threw ADT. |
