This report is a result of an off-site recovery house complaint investigation conducted on September 26-October 1, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the off-site complaint investigation, Greenhouse Recovery KW8R6701 was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

Observations
On October 1, 2025, DDAP staff conducted an onsite investigation which included phone interviews with staff and residents. This allegation was substantiated.Based on interviews with the resident and staff, the facility did not provide documentation of confirmation of discharge that included a summary of the resident ' s participation in Greenhouse Recovery according to the facility policy and procedure manual.

The Corrective Action: On October 2, 2025, we sent the completion of the program letter to the resident, officially discharging as a graduate who completed the program. A copy was placed in the resident's file. As noted the client was not a resident of DDAP Recovery House #KW8R6701
