This report is a result of an on-site complaint investigation conducted on 11/17/2016 by staff from the Division of Accountability and Program Improvement. Based on the findings of the on-site complaint investigation, Polaris Renewal Services, Inc was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

The nonresidential facility:
(2) May not permit in the facility heaters that are not permanently mounted or installed.

Observations
The facility failed to ensure that heaters that are not permanently mounted or installed were being operated in the facility.
During the 11/17/2016 on-site investigation, a portable heater was observed in use by one of the facility staff members.

Plan of Correction
The Executive Director provided a written memo and verbal training to all staff regarding the use of portable heaters. All staff were informed that the use of any type of portable heater is prohibited in the Polaris Renewal Services facility. The Executive Director also had an HVAC technician come to review the heating elements in areas of the building that were found to be extremely cold. The overall thermostat temperature was increased and the building heat was turned up. The Executive Director completed a spot check of all office spaces to ensure that all portable heaters were removed from the facility. |
