INITIAL COMMENTS

This report is a result of an onsite follow-up inspection regarding the plans of correction for the February 26, 2009 licensure renewal inspection. The follow-up inspection was conducted on June 9, 2009 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the onsite follow-up inspection, Health Masters Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on July 6, 2009.

Plan of Correction

709.24(d)  LICENSURE Treatment/Rehabilitation Management

709.24. Treatment/rehabilitation management. (d) Provisions shall be made, through written agreement with a licensed hospital or physician, for 24-hour emergency psychiatric and medical coverage.
Observations
Based on an interview with the facility director, the facility failed to provide documentation of a written agreement with a licensed hospital or physician, for 24 hour emergency psychiatric and medical coverage.

The findings include:

The facility director was interviewed on June 9, 2009. The facility director reported he has made numerous attempts, without success, to obtain a written agreement with Heritage Valley hospital. The facility was unable to provide a written agreement with a licensed hospital or physician for 24 hour emergency psychiatric and medical coverage, as required.

Plan of Correction
The Facility Director has corresponded with facility director of Staunton Clinic, a mental health provider with Heritage Valley Health System. A copy of previous agreement with now closed Aliquippa Hospital was forwarded to Staunton Clinic director, who responded that he will present agreement to Heritage Valley personnel responsible for agreements, and will work towards getting a signed agreement with hospital. Heritage Valley operates the two hospitals that are local to Health Masters and are the obvious choice for this agreement. Facility Director has also contacted the CEO of Primary Health Network who operates merdical and psychiatric care offices in the area. It is the Facility Directors intention to obtain an agreement with each of these providers. Facility Director will be responsible for following through with finalization of this agreement, and will work towards obtaining a signed agreement within the next 30 days, or Aug, 16, 2009
