The facility failed to obtain an informed and voluntary consent from the client prior to the disclosure of information in six of seven client records reviewed on March 7, 2017.
Client #1, #2, #3, #4, #5, and #6 did not include documentation of a release form for a funding source prior to the disclosure of information.
The findings were reviewed with the facility staff during the licensing process.

Plan of Correction
Beginning today, March 8, 2017, the Program Director will ensure that all clients have signed a Release of Information for ANY third-party recipient (including the Erie SCA) prior to the release of confidential material. Workers have been notified of this expectation and have already begun to have clients sign the appropriate releases. Release of Information compliance will be monitored each month during regular supervision by the Program Director. |
