This report is a result of an on-site licensure renewal inspection conducted on April 1 & 2 , 2008, by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Good Friends, Inc., was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on May 1, 2008.

709.53(a)(9) LICENSURE Aftercare plans
709.53. Client records.
(a) There shall be a complete client record on an individual which includes information relative to the client's involvement with the project. This shall include, but not be limited to, the following:

Observations
Based upon a review of client records on April 2, 2008, the facility failed to include an aftercare plan in one of two closed client records reviewed, client record #2, and failed to include the reentry process on the aftercare plan in client record #1.

Plan of Correction
a) The completion of aftercare plans will be monitored by the Clinical Director through chart review one week prior to client discharge and one week after client discharge. The Clinical Director will review the aftercare process and Observation #1809 with the clinical team members during an April 22, 2008 team meeting.
b) The Clinical Director will alter the aftercare form to include a written
re-entry process by April 21, 2008. The revised form and Observation #1809 will be reviewed with the clinical team members during an April 22, 2008 team meeting. |
