This report is a result of an on-site complaint investigation conducted on November 2, 2009 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site complaint investigation at Gaudenzia, Inc., Fountain Springs, there were no areas of non compliance identified with regard to the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
