Since that time therapist has provided documentation of that training and a copy has been placed in the staff training files. Three (3) copies of all training certificates will be submitted to program director. One will be placed in training file that is kept on the unit, one will be sent to Human Resources and one will be submitted to the Training and Staff Development Director to insure that there is sufficient documentation of all trainings.
