Client # 4 was admitted on December 11, 2025 and was discharged on January 9, 2026. The follow-up call was due to be completed by January 16, 2026; however; the follow-up contact/information was documented on February 5, 2026.

Client # 5 was admitted on August 14, 2025 and was discharged on October 10, 2025. The follow-up call was due to be completed by October 17, 2025; however; the follow-up contact/information was documented on October 24, 2025.

These findings were reviewed with facility staff during the licensing process.
