704.12(a)(6) LICENSURE OutPatient Caseload
704.12. Full-time equivalent (FTE) maximum client/staff and client/counselor ratios.
(a) General requirements. Projects shall be required to comply with the client/staff and client/counselor ratios in paragraphs (1)-(6) during primary care hours. These ratios refer to the total number of clients being treated including clients with diagnoses other than drug and alcohol addiction served in other facets of the project. Family units may be counted as one client.
(6) Outpatients. FTE counselor caseload for counseling in outpatient programs may not exceed 35 active clients.
|
Observations Based on the review of the Staffing Requirements Facility Summary Report, the facility failed to comply with the client/staff and client/counselor ratios during primary care hours. The FTE counselor caseload for counseling in outpatient programs may not exceed 35 active clients. The counselor Full Time Equivalent (FTE) is determined by dividing the total number of hours the counselor devotes to their clients by the facility operating hours. Then, in order to obtain the counselor ' s ratio, the total number of clients on the counselor ' s caseload is divided by the FTE.
Employee # 4 was hired as a counselor on January 17, 2020. Employee #4 was reported to have 5 hours per week devoted to their 5 clients on their caseload.
The FTE counselor ' s caseload calculation is as follows: 5/37.5 = .1333 (FTE); 5/.1333= 37.5093, which equals to a client/counselor ratio of 38:1.
Employee # 5 was hired as a counselor on August 16, 2019. Employee #5 was reported to have 1 hour per week devoted to their 1 client on their caseload.
The FTE counselor ' s caseload calculation is as follows: 1/37.5 = 37.5 (FTE); 1/37.5= 37.5, which equals to a client/counselor ratio of 38:1.
Employee # 8 was hired as a counselor on November 1, 2017. Employee #8 was reported to have 21 hours per week devoted to their 20 clients on their caseload.
The FTE counselor ' s caseload calculation is as follows: 21/37.5 = .56 (FTE); 20/.56= 35.7142, which equals to a client/counselor ratio of 36:1.
These findings were reviewed with facility staff during the licensing process.
|
Plan of Correction The Project Director will address in Drug and Alcohol Group Staffing the FTE Ratio requirements and have staff accommodate their caseloads to adhere to this ratio going forward. Employee #4, #5, and #8 caseloads will then be assessed in Individual Supervision with the Clinical Supervisor or Project Director to ensure that accommodations have been made. Caseloads will be within the FTE ratio by 10/15/2020 to ensure clinical care of clients. |
