This report is a result of a complaint investigation conducted on January 29, 2013 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against Gaudenzia West Shore were partially substantiated. However, the investigation confirmed that the facility had already evaluated and addressed issues in the complaint. Subsequently, there were no areas of noncompliance that were identified from the complaint investigation.
