Plan of Correction It is Libertae's policy to assist clients in administering their medications according to the regimen outlined by the prescriber. Whenever a client declines to take a medication that has been prescribed, this will be documented in the client's record at the time of the medication (pass) administration. Clinical staff shall follow up with the client in order to determine if a medication monitoring appointment needs to be made with the prescriber in order to address any issues of non-adherence to the medication regimen. Facility staff who are responsible for overseeing medication administration have been updated on this procedure. The supervisor of this staff will conduct random chart reviews on a monthly basis to ensure that the plan of correction has been completed and sustained. Any instance of staff neglecting to document these instances will be addressed through follow-up training. |

This deficiency will be addressed by the Clinical Director, who will review the need for timely updating of clinical treatment plans with the clinical team. 709.52(c) LICENSURE Provision of Counseling Services
709.52. Treatment and rehabilitation services.
(c) The project shall assure that counseling services are provided according to the individual treatment and rehabilitation plan.
|
Observations Based on a review of outpatient client records, the facility failed to ensure that the clients received counseling services according to their individual comprehensive treatment plan.
Client #1 was admitted on February 26, 2026, and was still active at the time of the inspection. The comprehensive treatment plan, dated January 30, 2026, indicated one individual session per week. A review of the client record did not include documentation of a weekly individual session during the week of March 22-28, 2026.
This finding was reviewed with facility staff during the complaint investigation.
|
Plan of Correction Within the individualized treatment plan, it is determined by the counselor and client the number of individual and group sessions the client will receive per week. The counselor will then document that these services are being provided, which must align with what has been determined in the treatment plan.
To address this deficit, Clinical Director will meet with the clinical staff to provide education on the need to ensure that the individual and group counseling services provided align with the number of individual and group counseling services that have been determined as necessary in the client's treatment plan. The Clinical Director will additionally conduct ongoing chart audits to determine that each client is receiving the identified number of sessions in one calendar week (Monday-Sunday).
