INITIAL COMMENTS

This report is a result of a complaint investigation conducted onJuly 16, 2020 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment.. Based on the findings of the complaint investigation, the allegations against Pyramid Inc. were unable to be substantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
