This report is a result of an on-site complaint investigation conducted on April 13, 2021, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, French Creek Recovery center was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

(b) A residential facility may operate a central food preparation area to provide food services to multiple facilities or locations. A residential facility that operates an onsite food preparation area or a central food preparation area shall:
(6) Store all food items off the floor.

Observations
Based on a physical plant inspection conducted on April 13, 2021, the facility failed to store all food items off the floor. One open box and two closed boxes of coffee were observed on the floor in the kitchen.

Plan of Correction
On 04/13/2021, the coffee was moved by the Director of Plant Operations. Staff members were retrained on proper food storage. Signage was placed to remind staff members to store food properly. Senior Leaders to complete rounding daily to ensure that food items are consistently stored per regulations within the facility. This documentation is reviewed by the CEO. |

(a) Exits.
(1) The residential facility shall:
(i) Ensure that stairways, hallways and exits from rooms and from the residential facility are unobstructed.

Observations
Based on a physical plant inspection conducted on April 13, 2021, the facility failed to ensure emergency exits were unobstructed. Outside the emergency exit to the left of the main enterance, there were a stack of lawn chairs, a trash can and a pump for a pond blocking the stairs. These stairs led from the emergency exit to the parking lot.

Plan of Correction
On 04/13/2021, the items were moved from the walkway by the Director of Plant Operations. Staff members were re-educated to keep walkways clear. The technicians complete a Security Rounds checklist each shift. Ensuring all walkways are unobstructed was added to this checklist. The checklist is submitted to the Director of Nursing each day for review. |
