INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on June 7, 2017 through June 8, 2017 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Gaudenzia, Inc. Fountain Springs was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

The following deficiencies were identified during this inspection:

Plan of Correction

705.6 (7)  LICENSURE Bathrooms.

705.6. Bathrooms. The residential facility shall: (7) Maintain each bathroom in a functional, clean and sanitary manner at all times.
Observations
The physical plant inspection was conducted on June 7, 2017. The facility failed to maintain each bathroom in a functional, clean and sanitary manner at all times.

At the time of the inspection, it was observed in the second floor bathroom that the cover of the lower portion of the heating unit was missing, exposing debris such as hairs, grass and touch-n-foam insulation. Additionally, without the cover, there were exposed, sharp edges, some of which were rusted.

It was observed in the stabilization unit bathroom that baseboards were missing, exposing crumbling gypsum, torn paper on the drywall and some areas with mold.

The findings were reviewed with facility staff during the licensing inspection.

Plan of Correction
The Facility Plant Manager will complete a physical walk through of the bathroom areas and ensure all necessary baseboards are replaced. In addition, the heating unit in the second floor bathroom will have a bottom panel added to it so as to cover the exposed edges and touch-n-foam insulation. The Program Director and Facility Plant Manager will oversee the completion of work to ensure the safety and well being of clients, children, staff and visitors. Expected date of completion for the above work is August 11,2017. Also, the Program Director will hold a mandatory staff meeting by July 30, 2017 to discuss conducting regular walk throughs of the bathroom areas in order to ensure sanitary and safe conditions. The Program Director will obtain written notification that this is understood.
