Observations
Based on a review of personnel records and an interview with the Director, the facility failed to document that a staff member, who is a counselor assistant, was working toward meeting the qualifications for the position of counselor.

The findings include:

Ten personnel records were reviewed on January 11, 2011. Four personnel records pertained to the position of counselor assistant. Per regulation, a counselor assistant shall document to the facility director that he is working toward counselor status. This information shall be documented upon completion of each calendar year. The facility failed to provide documentation that the counselor assistant is working toward meeting the qualifications for the position of counselor in staff record # 6.

Employee # 6 was hired on 11-09-2011. Employee # 6 did not provide documentation by December 31, 2010 that he is working toward counselor status. The documentation stated he was continuing his education in the Doctorate program in Hospital Administration and will not have time to attain his certified addiction counselor (CAC) certification in the time allotted by regulations.

The Director was interviewed on January 11, 2011 at 1:00 PM. The Director reported that employee # 6 has been attending college, working toward his Doctorate degree in Hospital Administration and not his CAC accreditation or counseling degree. His annual review stated that attaining his CAC was not possible in the next three years.
