Based on the review of client records, the facility failed to limit the release of information to what is permissible according to 4 Pa. Code 255.5 in one client record.
Client record #1 contained a consent to release information to a government agency, dated August 3, 2016, that permitted the release of the client's treatment plan.
The information was reviewed with facility staff.

Training will be conducted with all personnel on section 709.28, explicating the requirements. Training will be completed by 9/15/16. Responsible for training will be Project Director.
