Observations
A review of client records was completed on February 9, 2017. Based on this review, the facility failed to provide each patient an average of 2.5 hours of psychotherapy per month during the patient 's first 2 years of treatment .
Client #3 a current client, admitted on November 18, 2016, did not get the required 2.5 hours of treatment for the months of December 2016 and January 2017.
Client #7 a current client, admitted on November 23, 2016, did not get the required 2.5 hours of treatment for the month of December 2016.

Plan of Correction
Facility Director shall hold an in-service training for all clinical staff members related to required counseling hours and appropriate documentation of clinical interventions during counseling. Training will be completed by 3/31/17 and documentation will be maintained on site for review. Facility Director and Lead Counselor will monitor staff compliance through random chart audits and supervision. |
