INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on November 27 -29, 2012 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Gaudenzia Erie Inc. Corry Satellite office was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection.

705.24 (2) LICENSURE Bathrooms.
705.24. Bathrooms.
The nonresidential facility shall:
(2) Provide a sink, a wall mirror, an operable soap dispenser, and either individual paper towels or a mechanical dryer in each bathroom.

Observations
Based on observation and a physical plant inspection, the facility failed to provide soap in the bathroom.
The findings include:
A physical plant inspection was completed on November 28, 2012. Soap was not available in the bathroom at the time of the physical plant inspection.
The findings were reviewed with the facility director and were not disputed.

Plan of Correction
Program Director will review licensing requirements regarding the need for always having soap in the bathrooms with Corry Satellite staff and Office Manager by the end of the month.
Gaudenzia staff will check soap dispensers daily and notify office manager if soap is needed. |
