One out of three records, specifically record #3, did not contain the required documentation. Employee # 3 was hired as counselor on June 17, 2013. Training was due by June 17, 2014. As of the date of the licensing inspection, there was no documentation of HIV/AIDS training in record # 3. The TB/STD training for employee # 3 was late as it was not completed until July 15, 2014. These findings were reviewed with facility staff during the licensing process.
