Observations
The facility failed to provide individual counseling, at least twice weekly, and group counseling, at least twice weekly in three of three partial hospitalization client records reviewed during the course of the annual licensing inspection on June 21, 2016.
Client # 1 - Required counseling was not provided for the weeks of May 2 and 16, 2016.
Client # 2 - Required counseling was not provided for the weeks of May 9, 16, 23, and June 6, 2016.
Client # 3 - Required counseling was not provided for the week of June 6, 2016.
The findings were reviewed with the facility staff during the licensing process.
