Observations
The facility failed to ensure that all heaters are permanently mounted or installed as evidenced by the following:
An unmounted, portable space heater was observed on the floor of the Clinical Administrator's office during a complaint investigation on December 3, 2025.

Plan of Correction
The Project Director immediately removed the portable space heater from the Clinical Administrator's office and addressed staff on the use of portable units being prohibited. Moving forward, the Project Director will ensure that any heaters introduced into the facility are properly mounted or permanently installed in compliance with safety requirements. This correction is made effective immediately. |
