This report is a result of an on-site complaint investigation conducted on January 23, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Keep It Green was found not to be in non-compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

§ 717.29. Physical plant standards.
(b) Building interior, exterior and grounds. The licensee shall:
(3) Keep the drug and alcohol recovery house free of rodent and insect infestation.

Observations
Based on the physical plant investigation the recovery house failed to keep the drug and alcohol recovery house free of rodent and insect infestation

Plan of Correction
The house was treated by Akita Pest Control on Jan 31,2025 for both mice and roach control. The home was also placed on a quarterly treatment schedule with Akita. This will be monitored by the House Manager on a weekly basis for any new activity. There is a category for pest activity added to the Weekly Compliance Checklist that the House Manager uses for weekly house checks. The Compliance Checklist is turned in to the Director on a weekly basis. |
