The findings include:
Twenty two patient records were reviewed on February 6-7, 2013. Fourteen of the patient records reviewed were that of discharged patients. Two of the discharged patients were involuntarily terminated from treatment at the project. The facility failed to notify the patient, in writing, of a decision to involuntarily terminate the patient's treatment at the project in patient record # 13.
Patient # 13 was admitted into treatment on August 8, 2011 and was involuntarily terminated from the project on December 7, 2012. There was no documentation that the patient had been notified, in writing, of the decision to involuntarily terminate the patient's treatment at the project.
The Facility Director and Clinical Supervisor were interviewed on February 7, 2013 and confirmed the findings.

Plan of Correction The patient in question was terminated from the program due to the patient threatening to commit an act of violence in the clinic. When the incident occurred, the patient made the gesture and left the premesis directly after without staff interaction, and did not return.
In situations such as this in the future, Summit medical Staff will mail a letter of termination to the patient prior to discharge. This will allow the opportunity for the patient to appeal the dishcarge. Proof of mailing the letter will be kept in the chart.
The Facility Director, will convey this change in policy in the 2/19 staff meeting and this change will be effective at that time. |
