INITIAL COMMENTS

This report is a result of a complaint investigation conducted on July 15, 2010 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against Teen Challenge Training Center were substantiated. A plan of correction must be submitted by August 17, 2010.

Plan of Correction

705.1 (1)  LICENSURE Gen requirements for residential facilities.

705.1. General requirements for residential facilities. The residential facility shall: (1) Hold a license under Chapter 709 (relating to standards for licensure of freestanding treatment facilities) or a certificate under Chapter 711 (relating to standards for certification of treatment activities which are part of a health care facility).
Observations
Based on a physical plant inspection which included inspection of mattresses and pillows throughout the facility, as part of the onsite complaint investigation of July 15, 2010 at 7:30 A.M., the facility failed to ensure that all mattresses were in good condition.

Findings include:

Mattresses were randomly spot checked for cleanliness and good repair during the physical plant inspection . Two types of mattresses are used . One type is newer and has a shiny blue mattress cover made of durable fabric, the older type of mattress has a green vinyl/plastic cover. These latter mattresses were generally in worn condition. Tears were noted in mattress covers as follows: room #5 - one torn mattress cover; room #7 - one torn mattress cover; rooms # 9, #10 and dorm #1 each had an older type mattress with a cracked or torn mattress cover. Several rooms were spot checked to ascertain the condition of the mattresses. Several other rooms had mattresses that needed attention (such as zipped mattress covers) to ensure appropriate bedding.

Plan of Correction
On Saturday, July 31, 2010 a visual inspection was conducted by the Intake Personal, counseling and front desk staff. All mattresses were inspected. We immediately replaced mattresses. This included but not limited to rooms #5 7 9 & 10. Room 5 three mattresses were replaced. A total of 10 mattresses were replaced and an order for 240 mattresses is pending which will complete a 100% replacement of all mattresses.

To ensure this deficiency does not recur, staff working on Saturday's will require students to do a through cleaning of their rooms. As staff is making visual inspections of the room after they are cleaned they will make a visual inspection of all mattresses during this time. Only after the visual inspections are made shall the student make their beds. If a mattress has rips, tears or any other deficiency in them then front desk staff will call custodial department to have the mattress replaced.

To ensure this deficiency does not recur all mattresses are being replaced and rooms sprayed before new mattresses are installed. Teen Challenge has contracted with a local pest control company that will come and inspect monthly. Spray if needed. They are contracted to come when called if any problems arise.

705.9 (1)  LICENSURE General safety and emergency procedures.

705.9. General safety and emergency procedures. The residential facility shall: (1) Be free of rodent and insect infestation.
Observations
Based on an onsite investigation, the facility failed to be free of insect infestation.

The findings include:

Forty-one client rooms were inspected on July 15, 2010. After inspection of the rooms and a list provided by nursing staff, it was determined the facility has a bed bug infestation problem. Room # 1 Dorm (0), #8, #16, #5, #3, #30, #7, #8, and #3 are the rooms listed for Singleton Exterminator to spray on July 20, 2010. Interviews with clients living in #1 dorm (0) confirmed bed bugs were biting them and the staff is aware of this problem. Upon inspection of the rooms, a mature bug was located in room #10.

Plan of Correction
