Observations Based on the review of the Staffing Requirements Facility Summary Report, the facility failed to keep client/staff ratios at or below the regulation limit of 35/1.Employee # 1 was reported to have 10 hours per week devoted to the 10 clients on their caseload. The counselor Full Time Equivalent (FTE) is determined by dividing the total number of hours the employee devotes to their clients by the facility's work week. Then in order to obtain the employee's ratio, the total number of clients on the caseload is divided by the FTE. The FTE caseload calculation for Employee # 1 is as follows: 10/40 = .25; 10/.25 = 40. This equals to a client/counselor ratio of 40/1.

Plan of Correction When completing the Staffing Requirement Facility Summary Report, employee #1 will be given paperwork time to maintain and complete the required paperwork for her 10 clients. This will be effective whenever the form completion is being requested. All other hours will be documented under admin duties. Employee #1 will not have more than the allowable fulltime counselor to client ratio of 35 to 1. |
