The resident may revoke consent to provide notice to the emergency contact. (b) The licensee shall make at least one attempt to notify the resident ' s emergency contact in accordance with subsection (a) immediately and in no event more than 12 hours after the resident decides to end residency or does not return to the drug and alcohol recovery house as expected. This subsection shall not apply if the licensee knows or has reason to know of allegations of domestic abuse on the resident by the emergency contact.
Observations
Based on a review of resident records, the recovery house failed to make at least one attempt to notify the resident ' s emergency contact in accordance with subsection (b) immediately and in no event more than 12 hours after the resident decides to end residency or does not return to the drug and alcohol recovery house as expected.

These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
Invision will make an effort to notify a resident's emergency contact within 12 hours of a resident deciding to end their residency. This contact will be documented in the resident's file upon completion. This policy was relayed to all staff and volunteers in person on 9/30/2024, and a space to document this contact was added to the Consent to Residency form. Upon receiving notice of a resident's decision to end their residency, a staff member will attempt to make contact with their emergency contact. This contact attempt will be documented on the resident's Consent to Residency form whether or not any attempts at contact were successful.

§ 717.31(a)(6)  LICENSURE Unusual incidents - Incidents

§ 717.31. Unusual incidents. (a) The licensee shall develop and implement written policies and procedures to respond to the following unusual incidents: (6) Event at the drug and alcohol recovery house requiring the presence of police, fire or ambulance personnel.
Observations
Based on a review of resident records the recovery house failed to file a unusual incident report to the Department within 3 days of an event that resulted in the presence of police, fire or ambulance personnel.

These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
It is the policy of Invision Recovery Houses that unusual incidents are being submitted for any police or fire presence at the recovery house. Upon learning of police or fire presence at the recovery house, the Residential Supervisor will gather all relevant information and submit an unusual incident report within 3 days of the incident occurring. This policy was reviewed with the Residential Supervisor responsible for submitting the unusual incident reports on 9/30/2024.
