Observations
Based on a review of the Staffing Requirements Facility Summary Report (SRFSR) and personnel records, the facility failed to document the completion of 25 clock hours of annual training required for counselors in employee #4's file.

Employee #4 was hired as a counselor on May 6, 2019 and was still in the position as of the date of the onsite inspection. The facility's training year that was reviewed was from January 1, 2020 through December 31, 2020. Employee #4's employee record only documented 8 hours of annual training for the training year reviewed.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
On 9/27/21, Facility Director met with Employee #4 to discuss the importance of completing 25 hours of annual training. During this coaching session, the Facility Director reviewed the various ways that employee #4 can schedule and attend specific trainings needed for employment. In order to ensure that Axiom Family Counseling Inc., remains compliant with this regulation, the Facility Director will review with all current staff the importance of maintaining the minimum amount of 25 hours per year of job-related trainings. The Facility Director will review with the staff during individual and group supervision.
