This report is a result of an on-site complaint investigation conducted on March 6, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Polaris Detox LLC dba Innovo Detox was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

705.2. Building exterior and grounds.
The residential facility shall:
(2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.

Observations
During a complaint investigation conducted on March 6, 2025, the facility failed to keep the grounds of the facility in good repair at all times.
Three chairs were observed to be in disrepair in the Conference Room. One chair had cracked leather-like material on the seat and it ' s arm cushions had duct tape holding them onto the chair. There were two other chairs with the outside plastic broken off the arm cushions.

Plan of Correction
The three chairs were discarded, and new ones were ordered. All common areas, including the conference room, have been added to the environment of care checklist, which is checked monthly to ensure compliance with the highest standards. The maintenance supervisor conducts the monthly checks and submits the results to the facility director for review. |
