(c) Clinical supervisors and lead counselors who have not functioned for 2 years as supervisors in the provision of clinical services shall complete a core curriculum in clinical supervision. Observations
Based on the review of three personnel records, the project failed to ensure that a clinical supervisor that has not functioned for two years as a supervisor in the provision of clinical services completed a core curriculum in clinical supervision.Employee #2 was hired as a clinical supervisor on August 2, 2025, and was current in that position at the time of the inspection. Employee #2 had not functioned for two-years as a supervisor in the provision of clinical services and as of the time of the inspection had not completed a core curriculum in clinical supervision. These findings were reviewed with project staff during the licensing process.

Plan of Correction
1) Employee #2 will complete the required 30 hours of clinical supervision training with an approved DDAP provider for this training in accordance with state regulations (Clinical Supervision Series Part 1/2 Havana Wellness Studio). This training will address all required components of the core curriculum for clinical supervision to ensure compliance with regulatory standards.

2) The agency will implement a standardized pre-employment and role-transition verification process requiring documentation of completed supervision training prior to assuming or continuing in a clinical supervisor role. Human Resources and Program Leadership will review supervisory credentials upon hire, promotion, and annually thereafter to ensure ongoing compliance.

3) Employee #2 started the required 30-hour supervision training on 03/02/2026, with scheduled completion no later than 04/30/2026. All corrective actions related to Employee #2 will be completed by 04/30/2026.

4) Documentation of training completion will be placed in Employee #2's personnel file. Compliance will be confirmed by the Project Director.

5) The project Director is responsible for ensuring that this corrective action plan is fully implemented and monitored.
