This report is a result of an on-site complaint investigation conducted on 07/11/2024 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Another Day Clean was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717 which pertain to the facility.

§ 717.29(b)(1) LICENSURE Physical plant standards - Bldg-Exterior-Grnd
§ 717.29. Physical plant standards.
(b) Building interior, exterior and grounds. The licensee shall:
(1) Keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times.

Observations
Based onsite complaint investigation the recovery house failed to keep the drug and alcohol recovery house and free of hazards at all times
These finding was reviewed with the recovery house.

Plan of Correction
the step is scheduled to be repaired by the property management company, City Line Construction. I will send pictures of repaired step once It is complete. I submitted maintenance requests on 7/8/24 and 7/16/24 and got no response. I called them today (7/18/24) at 1112am and let them know I need this addressed immediately. I forwarded the email to Ashlee Jones and the property management company stated the repair will be done within a week. In the meantime, we have It roped off and blocked with an orange cone. We will do our best to ensure It will not recur, but cannot guarantee since It is a concrete step and they will sometimes have damage from general wear and tear. |
