INITIAL COMMENTS
This report is a result of an off-site complaint investigation conducted on October 3, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the off-site complaint investigation, Pyramid Healthcare Lehigh Valley was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.	Plan of Correction
709.34 (c) (4) LICENSURE Reporting of unusual incidents § 709.34. Reporting of unusual incidents. (c) To the extent permitted by State and Federal confidentiality laws, the project shall file a written unusual incident report with the Department within 3 business days following an unusual incident involving: (4) Event at the facility requiring the presence of police, fire or ambulance personnel.
Observations Based on a review of client charts, and administrative documentation, the facility failed to file a written unusual incident report with the Department within 3 business days following an unusual incident involving an event requiring the presence of police on August 13, 2025.	Plan of Correction Executive Director met with Administrative Assistant to review the importance of timely submission of external incident reports. Starting 10/17/2025, ED will follow-up with Administrative Assistant within 24 hours of significant incident to ensure timely submission of EIR. If AA is out of the office, ED will complete EIR within 3 business days of the event. In the absence of ED, site leadership such as Clinical Director (CD) will complete the documentation.
