This report is a result of an initial on-site physical plant inspection. The inspection was conducted on November 23, 2010 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the physical plant inspection, Axiom Family Counseling, Inc., was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility, specifically 28 Pa. Code Subsection 705.1. Therefore, no deficiencies were identified during this inspection and no plan of correction is required. Since this was an initial inspection, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.
