INITIAL COMMENTS

This report is a result of an onsite follow-up inspection regarding the plans of correction for the November 28, 2011 through December 1, 2011 licensure renewal inspection. The follow-up inspection was conducted on September 13, 2012 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the onsite follow-up inspection, Kirkbride Center was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection.

Plan of Correction

704.7(b)  LICENSURE Counselor Qualifications

704.7. Qualifications for the position of counselor. (3) A Bachelor's Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 1 year of clinical experience (a minimum of 1,820 hours) in a health or human service agency, preferably in a drug and alcohol setting. Observations
Based on the review personnel records, the facility failed to document that each counselor met the qualifications for the position in one personnel records reviewed.

The findings included:

Eleven personnel records were reviewed on September 13, 2012. One counselor personnel record was reviewed. The facility failed to document qualifications for counselor in one personnel record, #8.

A review of personnel record # 8 revealed that they were not demoted to counselor assistant from counselor since the last inspection. Personnel #8 at the time of the inspection last year, November 28, 2011, had no documentation that she had any experience for the position based on a review of the employee's application that listed less than a year of clinical experience with a Bachelor's degree in a related field.

The findings were reviewed with facility staff and the findings were not disputed.

Plan of Correction
1. The status of Employee #8 was changed on September 14, 2012, from Counselor to Counselor Assistant.

2. Employee #8 is supervised by the facility's Clinical Director and he has initiated direct and close supervision for a period of six months.

3. Although Employee #8 has a master's degree in business, the master's degree does not count toward education for the purposes of being a Counselor Assistant. Therefore, the facility has recognized the employee's Bachelor's degree in Sociology, with a concentration in Criminal Justice. The Clinical Director will continue to document close and direct supervision for a period of six months, as per state standards for a Counselor's Assistant with a bachelor's degree. At that time, the employee will be considered for promotion to Counselor status.

704.11. Staff development program. (a) Components. The project director shall develop a comprehensive staff development program for agency personnel including policies and procedures for the program indicating who is responsible and the time frames for completion of the following components: (1) An assessment of staff training needs.
Observations

The Project Director was interviewed on November 30, 2011. The Project Director confirmed that a needs assessment was not completed on each support staff member for the 2011 training year.

This deficiency remained out of compliance at the time of the follow-up conducted on September 13, 2012.

Plan of Correction
1. The facility has hired a Human Resources consultant to complete several projects.

2. Among the projects is completion of the Individual Training Plans for all support staff.

3. The consultant will make a status report on the Individual Training Plans to the Clinical Management Team by October 31, 2012. The purpose of the status report is to ensure the team provides oversight and monitoring to the process.

704.11(b)(1)  LICENSURE Individual training plan.

704.11. Staff development program. (b) Individual training plan. (1) A written individual training plan for each employee, appropriate to that employee's skill level, shall be developed annually with input from both the employee and the supervisor.
Observations
Based on the review of the staffing requirements facility summary report and personnel records, the facility failed to provide documentation of individual training plans in 48 out of 69 personnel records.

The findings included:

The Project Director was informed of the findings. The findings were not disputed.

This deficiency remained out of compliance at the time of the follow-up conducted on September 13, 2012.
