This report is a result of an renewal on-site licensure inspection conducted on March 10, 2026 through March 12, 2026 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection, LK2D6701 TNH Sober Living was found to not be in compliance with the applicable chapters of 28 PA Code, Chapter 717 which pertains to Recovery House licensing.

§ 717.30(b)(1) LICENSURE Safety and emergency procedures - Exits
§ 717.30. Safety and emergency procedures.
(b) Exits. The licensee shall:
(1) Ensure that stairways, hallways and exits from rooms and from the drug and alcohol recovery house are unobstructed.

Observations
Based on an inspection of the physical plant, the recovery house failed to ensure all exits from the drug and alcohol recovery house remained unobstructed at all times. These findings were reviewed with the recovery house staff during the renewal inspection process.

Plan of Correction
The Executive Director/House Manager communicated the need for the impacted exit(s) to be unobstructed immediately to the Resident Lead. The Resident Lead removed the obstacle and informed This was verified by the Executive Director/House Manager.
At the Resident Leads Meeting on 3/18/2026 the Executive Director informed the Resident Leads of the expectation that they do a walkthrough of the entire residence daily to ensure no violations of policy or of safety standards go unnoticed and uncorrected.
The Lancaster District Manager will complete monthly rounds of all houses to identify safety and/or physical plant concerns, beginning 3/16/2026. Identified concerns will be reported via the Maintenance Ticketing System. |
